Health Consortium of Greater San Gabriel Valley

The Health Consortium of Greater San Gabriel Valley (HC-SGV), a partner of Heluna Health, strengthens the healthcare safety net and improves access to high-quality physical, behavioral, and substance use disorder services across the Greater San Gabriel Valley. Established in 2000, HC-SGV is a collaborative of community health centers, hospitals, public health departments, universities, and nonprofit organizations working to coordinate care and optimize service delivery.
